At Tri-State Family Eyecare, we understand that each patient has a different story, so we take the time to learn about our patients to better cater to their eye care needs. If this is your first visit, allow an hour or two. That includes time to get the exam and to get fitted for a prescription if you need it. Later appointments won't take as much time but they can vary depending on the reason for your visit.

- Aim to arrive 15 minutes before your appointment to either fill out or update your patient information. Note: only the scheduled patient is allowed in the office. If the patient is a minor or needs assistance only one caregiver will be allowed to accompany the patient.
- Review and follow our COVID procedures.
- Be prepared to answer questions relating to any eye health problems you have experienced, past and present. These can include:
- When your eye or vision symptoms began
- What medications you are taking
- Any work-related or environmental conditions that may be affecting your vision.
- Any previous eye or health conditions you and your family members have experienced.
- History of health and eye issues in your family
Plan To Bring the following: - Health and Vision insurance cards and information
- Your primary doctor's name and telephone number
- Your healthcare information, including any allergies. This will expedite the process and make filling paperwork out easier. If you have access to our patient portal, you can update your patient record online.
- A list of any questions you have about your eye health
- A list of medications and supplements you are currently taking (even the ones that may not seem relevant to your eyesight).
- Your current eye wear and prescription, if you wear them - this includes contacts and eyeglasses you use daily, reading glasses and sunglasses.
- Your family's eye history
- Your payment or co-payment. We accept cash and all major credit cards. Checks can be used for down payments.
